Register with our practice

Please print off both registration forms (part 1&2), complete and bring them both to the Surgery when you come in to register. You can come into register at any time during surgery opening hours – 8am – 6.30pm Monday – Friday

Please bring 2 forms of ID to include proof of address and photo.

When you register you will be allocated a named,accountable GP.

If you have repeat prescriptions you will be able to order by email or online once registered for Online Services – for details please click on Prescription page.

Catchment Area (Practice Boundary)

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

Due to contractual requirements all patients registered at the Practice must have an allocated accountable GP. We currently operate this practice but it will now be recorded in your medical record.

Access to Health Records

Patients can request access to their Full Medical Records by either

1. If you are signed up to online services you can request Full Clinical Access to your record by ticking the appropriate button. This sends the request to your GP for review. If the request is granted you will be able to access your records or if declined you will receive a message. The process can take up to 4 weeks.

2. You can visit the Practice in person and complete an Application for Access to Medical records form held at Reception. Once completed it is processed by the Practice Manager. If your application is granted, we will arrange for paper copies to be done. This process can take 2 weeks.

We would like to encourage the online service method if your request is not Urgent to reduce paper use.
