- Wanting a lifestyle change g. smoking/weight
- Joining/ accessing community clubs

Social Prescribing at Alma Road Surgery
Self referring to Physiotherapy
Hampshire and Isle of Wight Healthcare NHS Foundation Trust operate across Hampshire and the easiest and quickest way to access our Physiotherapy services is to self-refer.
To self-refer to Physiotherapy you must:
- Be aged 16 or over
- Be registered with a Portsmouth, Southampton or Eastleigh Southern Parish surgery
- Have a musculoskeletal (MSK) pain/condition
